Admissions Process

Admissions are accepted 24 hours a day, 7 days a week. We try to make this as easy and seamless as possible. The documents needed prior to admission include:


*Medicare card

 

*Medicaid card


*MedicareD Prescription card


*Insurance cards (Supplemental and Long-term)


*Social Security card


*Power of Attorney papers (HCPOA & FPOA)


*Living Will


- We are willing to make copies of these important papers for you during the admission.


We recommend a resident bring seven complete outfits and shoes should have non-slip soles. Laundry services are provided free of charge by the facility. Each personal item should be marked in permanent ink with the residents name. We pride ourselves on having a home like atmosphere so families are encouraged to bring pictures, plants, mementos and even a favorite chair. We discourage valuables like expensive jewelry and a lot of cash. We do have a safe in the front office if a resident wishes to have items such as these locked up.

Our facility is able to work with most payor sources. If you have questions about your eligibility for any of the following please call, we can help. In some cases we can help a family sign-up for programs they did not know were available to them.
