Admissions Process

Terrace Oaks Care and Rehab Center

We accept admissions 24 hours a day, 7 days a week. We strive to make the admissions process as smooth as possible.

The documents that are needed prior to admission include:

*Medicare card

*Medicaid card

*Medicare D Prescription card

*Social Security card

*Power of Attorney papers

*Living Will

*Any other Medical insurance (VIVA, Medicare Complete etc)

We will make copies for you during this process.

Laundry services are provided free of charge by the facility unless otherwise specified. The resident will need seven changes of clothes and nonskid shoes. ALL personal items should be marked in permanent ink with the resident’s name. We encourage you to bring in personal items to make the residents room as much like home as possible. We do discourage you from bringing valuables.
